Please complete the following screening questions to determine if you may be eligible for this study.

1. Have you been diagnosed by a physician with any of the following? *This question is required.

Type 1 Diabetes (typically childhood onset requiring insulin therapy)
Type 2 Diabetes-NOT requiring insulin injections (typically adult onset beginning with oral treatment and progressing to insulin therapy)
Type 2 Diabetes- requiring insulin injections (typically adult onset beginning with oral treatment and progressing to insulin therapy)
I have diabetes, but don't know if it is Type 1 or Type 2
I do not have diabetes
